I live outside the USA (in Vietnam), and have never ever had a single Bioware purchase go through smoothly.
This is over the history of Dragon Age, Origins, DA2, Mass effect 1 and Mass effect 2.
I have All the DLC for all the games and in every circumstance it was a struggle, where I had to end up using my Australian debit card.
The issue is not with my banks in Vietnam, the card is unlocked has been used successfully with many overseas institutions, the problem only applies to buying Bioware points. There is no block on the card or bioware's site, and EA/Bioware technical support offer no solution
What do the rest of you do if living outside the USA?
In the past I used my Australian visa debit card, but it's finally run out of money, and i need to use my Vietnamese debit card. It seems like they automatically reject transactions from Vietnam, although EA deny this.
Any experiences or advice anyone wants to share?

You might get more luck asking in the Bioware Points Forum.
Click on Forum Home, scroll down to General, and Bioware Points is under there.

Apart from that, I can only suggest trying out Paypal.
