If I have a ring equiped that adds +3 physical damage with zero stars compared to same ring but with a star rating what is the difference? Just vendor price?

The stars indicate the item's usefulness/benefit to the active character (based on current level or class). You'll see the stars on a specific item decrease as that character levels up - meaning that you could probably due with swapping it out and finding something that would be more effective.

There's a hidden item level that affects the item price and star rating. Otherwise there's no difference. Star rating is best ignored. I think it was intended to dumb down itemization, but it's not useful for deciding what to wear.

Ah yes that was the clarification I needed. I wasn't sure if I stopped receiving the benefit from a zero star +3 physical damage ring and say a 2 stared one.

Yeah, the item level is set when you acquire the item. An item you acquire later will have a higher 'item level' so that even if the stats it provides are exactly the same, it might have a higher star rating. Best just to compare item stats directly.

It wasnt intended to dumb down anything because it didnt remove any information, it simply adds a simplistic evaluation for people who would prefer that their inventory be dumb.
Advanced users can still read the descriptions and make their own valuations and choices.

And if you happen to use a mod that raises the level cap, then by thend of act 1 everything is no or one star just because of your level. I prefer less xp per level and gaining skills faster, but then I prefer nightmare mode.

How I read your post: "It wasn't intended to dumb down anything, it simply dumbs it down." LOL. "Itemization" to me can mean the choices you make with the items you have. Not removing information doesn't preclude the star rating from being an attempt to dumb down the process of deciding what to do with your items.
