So I just finished the game for the first time and found out there are 2 people I never met.
When do you meet Sebastian Vael and Isabela?

Isabella is in the hanged man during Act 1. I don't know about the other one.

That is very strange, because I can't see how you can miss them. They're both in places where you have to go or cross for the main plot to carry on.

To meet Isabela, you need to first recruit Anders. Then when you next go to the Hanged Man (at night), you should get her cutscene.
For Sebastian, you need the Exiled Prince DLC, then go to the Chantry in Act 1 during the day. You should see the cutscene that introduces him.

Yes Sebastian is hard to miss but Isabela ... well many people missed here - and it is very easy.
SPOILER !!!
She apears in Hanged Man only after you meet with Anders - depending on how you are playing your game you can do that just before you go to deeproads so you can easily miss her. And she is no longer available after you return from expedition.
