My elf, Shaia, fits that description.  The way I did her was to create the tattoo in Photoshop (using some of the game's default tattoo markings as the base resources) - two of the four layers for each color, otherwise you can't get very vivid coloring on them. 

Then just make the face in the toolset (as mentioned above, tutorials abound), apply the custom tattoo, and away you go.

Gimp is free and also has the capability to edit .dds files. And I'm sure there are other programs available.

i dont suppose anyones willing to do the hard work and make several combined or new tatoosand put themto a mod?
