Honestly, there should be a heavy achievement for getting Shepard to die... it would be the most difficult one to get from all ME2 achievements.

How is it difficult to not do anything?  

Shepard: I wanna go the Omega 4 relay NAO!

Shepard: Freedom's Progress is on it's way to Omega 4 relay?  We go nao!?!
TIM: I spent billions of credits bring you back-

When Shepard dies no save game is created if I remember correctly, so the question (and all the subsequent discussion) is moot. There is no "dead Shepard" save game to import to ME3.

This has already been talked about. Your character and file will not be transferred to ME3. The closest thing to that though would be to let all your crew die except 1 or more people though.

Which for absolute certainty includes ensuring none of Garrus, Zaeed or Grunt are on the HtL otherwise Shep will survive (because they'll ensure at least 1 other squadmate survives) well, unless you want crew dead as well, in which case you can have one of them there as long as the other 3 are Tali, Kasumi and Mordin

Import a dead Shepard and you unlock zombie outfits for all your squad-mates
