I totally thought she'd have a larger role in DA2 based on how much she was in the pre-release advertising things. And then it was just like "Well okay why are you even in this game?"

Also why would she change her outfit within the same time frame?

I would imagine that over the years (centuries?), Flemeth has been threatened by many, and has had to take precautions to protect herself. And knowing Morrigan as she did, she probably figured that her daughter would eventually attempt to take the grimoire in some fashion. I don't have the exact breakdown by years (and I'll admit to being too bleary-eyed to look it up now), but I think Hawke's first encounter with Flemeth would be before the episode with Morrigan and the grimoire quest. However, Hawke's second encounter with Flemeth was well after that.

On a side not I also thought of it in that way that syhe saved a piece of her here and there and shout she be "killed" she just reform.

There were various sources to tell you that killing Flemeth wasn't permanent. The codex implied she wasn't permanently dead, Morrigan believes she'll come back again once you give her the grimoire so she prepares, she says in Witch Hunt that her mother has tricked her way past death before, etc..
Flemeth is a key piece to the DA story. Bioware intended for her to matter, so they took steps to ensure that she could be available without the fanbase crying out "It's a retcon."

Oh! You mean is it canon? If you can find part of it where something actually happens, sure. I still recommend the heavy artillery though.
As for Flemeth dying, I don't know how anyone could've taken her death in DAO seriously. They pretty much wrote "she's not really dead" on a sign and beat us with it. It wasn't subtle. People can complain about Leliana or Anders being back from the dead, but they were very upfront about Flemeth's defeat being little more than an annoyance to her.

Our oft-dragon shaped Asha'Bellenar has a history of saving powerful (or soon to be) heroes (or anti-heroes as the case may be) ((Marric and Loghain, Warden and Alistair, Hawke and Carver). So Andraste - as a powerful mage who incited a rebellion and toppled an oppressive empire is VERY much like the usual persons Flemeth saves. Andraste could easily have been one of the many daughters of Flemeth. Funnily enough, making Andraste Morrigan's "sister"... tis interesting at least

I wasn't surprised either when Flemeth popped out of the amulet on Sundermount. My clue was freeing the spirit in the life gem during the Nature of the Beast quest in Origins.
@ Devilsgrin. I've thought of that myself that she was Andraste's mother or she is Andraste. Nobody really knows who or what Flemeth actually is. We do know that is strongly connected to the Dalish Elves and she holds some type of title with them.
