Forgot to mention the mall in Condemned.
Those mannequins, and the BGM. Christ.
Too bad the stupid sequel had to ruin the franchise with its 'magical powers' and cults nonsense.
I didn't get beyond the first or second chapter in Condemned 2. Mostly because it was a rental (just like Condemned) and I couldn't rent it any more since. Tell me about how they introduced magic and cult-related stuff? I saw some cult-stuff in the first, but that was due to the sonic-noise-sickness thing, which I felt was pretty cool.

343 Guilty Spark from Halo was pretty creepy in setting up the Flood's entrance to the game.
Alan Wake was also good at making me nervous any time the fog closed in and the wind picked up while I was lost in the woods.

I'd also like to add that Dead Space was so scary to me, after the intro and first encounter with the necromorphs I didn't want to leave the elevator and immediately shut off the xbox and started watching American Dad to calm myself down. I kept looking behind me and up at the AC vents the rest of the day.

Anyways since this is one of the few times I wholeheartedly agree with a games reviewer I'll let Ben "Yahtzee" Croshaw summarise it for you in a much more entertaining fashion.
*EDIT: Just checked, apparently if you completed the first game 100% it'd reveal a cult was behind it. I am dissapoint.

Altough I think Amnesia: The Dark Descent might finally dethrone it. Just bought this game from the steam summer sales, and giving it a go soon. Heard "good" things about it...

I also found the underwater base from Deus Ex pretty scary. One got so used to the relative freedom of levels in DE and to the abundance of human non-combatant NPCs - to suddenly find oneself under the sea on flooded building with nothing except corpses and monsters for company...it was very atmospheric.
