My first playthrough is almost always on casual, mostly because I only get games in which the story interests me and I want to hurry up and see how it plays out. Actually appreciating and experimenting with the gameplay comes later. If a game interests me enough for multiple replays, I'll eventually end up on Hard/Insanity/Whatever.

I usually start on a normal difficulty if I'm playing a new type of game I've never played before, but I usually go to the hard/nightmare difficult soon after becoming familiar with the controls.

For the record, I usually start on the average (not medium, but the average. There's a difference) difficulty and go up from there. For DA 2 I started at hard because I new normal was toned down from DA:O.

I usually do my first playthroughs of games on casual to rush through story content, then it's nightmare mode forever if I actually like the gameplay.

I play first and foremost for the characters and story and I always play on default the very first time and then go up - one level in one playthough. Considering I played through DA 2 six times, it is pretty much only Nightmare for me now.

I'm on the highest difficulty possible in whatever particular game I may be playing, because much of my fun with gaming (and many things in life as a whole) comes from the reward of learning, strategizing, putting ideas to the test, and excelling. If I do decide to invest my time into something, and have a genuine interest in it, I'm not stopping until I know absolutely everything about it, and can push it to the absolute limits. I drift into dangerous and unhealthy levels of perfectionism, but it's mostly how I have my fun.
With Dragon Age 2, the story was pretty much a bust for me, but I loved the combat, so I really got into it and enjoyed trying all kinds of stuff out, testing out theories, and finding the most awesome, powerful, fun ways to play it. Despite my current dislike for what has been done for the game, and the terrible things I sometimes say about it, I really had a good time in doing it.
I'm aware that this is an RPG game, and most are playing it to mainly experience the story, so I wrote guides and made videos of what I learned to help those people (and the other min-maxing nutjobs) get more out of it, and have a better time with it. I've really had a great time helping people out and interacting with the community, it's definitely given the game much more life than it would've had on its own.

I always put it on the highest difficulty and lower it if necessary. I never lowered it in Origins (Except when "speed running" to make more imports right before/after DA2 came out). I played Mass Effect (1) on insanity execpt when once again speed running for ME2. I played ME2 only on Insanity period. I tried to start DA2 on Nightmare but the friendly fire makes it almost unplayably boring. You have to castrate your mages or they will kill your whole team because one winters grasp from Anders can insta kill a teammate but hardly does anything to an opponent. They have so much health it makes Nightmare a boring "Everyone DPS and Anders heal" fest which I grow tired of really fast. After my first (and only NIghtmare run) I just switched it to hard.

The game was so ugly and stupid that i lowered difficulty to easy just to rush through in short time and see the end of it.
Difficulty level, however, changed only the number of waves and the health bar of enemies.

I thought hardcore gamers were by definition the ones who played on hardest difficulty setting and then added extra challenges for themselves (eg. 100% completion, no healing items, weakest weapon only, etc)

And I think that the "normal" difficulty is the one on which we should base our judgement when talking about difficulty.
You know that casual is supposed to be easy and hardcore is supposed to be hard.
But Normal is supposed to be BALANCED, and that is much more difficult to achieve than making something extremely easy or extremely hard.

Some of my favorite games don't allow you to switch difficulties halfway through the campaign, and on those I always start with at least easy difficulty (especially if the combat or franchise is unfamiliar). Otherwise I play on Normal, and over the years I have grown accustomed to deciding whether the amount of challenge is too much or too little, and I change accordingly.

I frequent the Normals, sometimes Easy, or Hard, depending on how things go. I think Crysis + Warhead are the only games I ever finished on the hardest difficulty setting, that I can remember anyway.
If you asked me to play Nightmare on DA 2, I'd rather watch paint dry.

Upping the difficulty just really mean gimping your character and not raising the intelligence of the NPC.
Monster just really spawn 2 time faster , hit 2 time harder and you have 2 time less Hitpoint.
When the difficulty is really worth to raise than yes.For instance when the game is really to easy .
But i personnaly prefeer handicaping myself rather than upping the difficulty . For instance i refuse to wear any retardly overpowered equipement or playthrough the game using only the overpowered stuff.
Like some ppl saying - OMG THIS GAME WAAYY TO EASY(But i wear the omg bbq dlc dragon armor at lvl 1(

Normal or hard. I play DA2 on hard though because i like challenge, long battles with tactics and sometimes frustration.

I play on the default my first playthrough, and only turn it down if battles are annoyingly hard, but then turn it back up right after I get past that part (rock wraith, arishok). Then on my second playthrough, I try it on hard, since I have a better handle on everything.
Have yet to attempt either Origins or DA2 on Nightmare. Mostly cos, well, after I finish my 2nd playthrough on Hard, all my other playthroughs were mostly to see what other story branches I can get, so I don't really want to challenge myself and get frustrated at that point.

I normally play one difficulty level above normal, sometimes I up it to hardest but it kind of depends on how the system works. Most of them are just bad and have the FO3 difficulty syndrome (the game is not hard but it require you to empty three pistol mags in someones head to kill them), and some works like DA2 where you kill your companions in 2sec from friendly fire but you only scratch the enemy for 1% of their hp.

I consider myself a "hardcore gamer" only in the sense that I love video games of all types and play a lot.... not because I play everything on hard. Maybe we need to define the term...

DA2 is normal/default all the way, though I should turn it up since I find myself multi-tasking (play game + talk on phone +surf the web at the same time!)

I prefer the game to force me to use the tools i have at my disposal, and that tends to happen at a notch above the default. So that's what i normally play these games at, it's usually labelled "hard" or something similar. Below it becomes too easy and as such boring, above it may require min-maxing which i dislike (preferring "builds" which reflect the idea i have about the character's past and ongoing experiences that shape them)

The exceptions are games that restrict saves on the hardest difficulty or you only get 1 life ever for the whole game (e.g. Dead Space 2, The Witcher 2) - if this kind of difficulty exists in a game, I'll start out one below it so I can plan out how to handle it.

If i like the game i'll play on hard. I want to feel the dread of going into a fight and have no idea if i will come out alive or not. And sometimes i switch it up another notch for boss fights.
