I am sure everyone has their opinions about this. For me I found Garrus every useful and also Miranda. Tali was good anywhere with robotic enemies.

Miri. I find she is useful in almost every situation. Most squadmates have their strengths and weaknesses, though.

On Insanity....Thane and Kasumi....Thane's warp and sniping shines on Insanity...Kasumi is all purpose, shadows herself out of trouble and her flash bangs are a god send.
Then Garrus, Legion and Tali, depending on who your battling.

Kasumi's Flashbang, and Shadow Strike have gotten me out of tough situations, her Overload is a bonus.Garrus' sniping and Armor piercing ammo is beast... but for me he just...won't...stay...in...COVER!!!

Miranda.

Miranda, hands down. Sentinels are always slightly overpowered with both warp and overload. Also, she has increased weapon damage for the whole squad...
Really good to have her + Garrus imo, for overall best control on insanity in ME2.

In ME1 Liara because of her cc biotics + overload.
In ME2 Miranda because she has warp and overload to deal with any enemy protection.
Tali is great against Geth in both games.

It's inevitably going to depend on what class your Shepard is, I would assume. My infiltrator Shepard tended to find biotics useful, for warp and tossing husks around. My Vanguard Shepard liked snipers for picking off annoying 'standing on an unreachable ledge, sucks to be you' enemies.
I personally would find folk like Jacob or Thane a lot more useful if they didn't insist on replacing their squad warp/armour percing ammo with their comparatively useless equivalent powers. Shredder ammo makes me weep. Stop activating it, I'm begging you.

Miranda easily

on Insanity i have to choose wisely my squad mates, but Miranda is the most universal. With her Warp and Overload she can breach any defense. Also Kasumi's shadow strike is useful as well, but sometimes a little slow...

Miranda by far. She can take care of anything you will ever meet. Overload+Warp solves a lot and then i use Garrus and shields gets taken care of fast!

Miranda, hands down.
In ME1 probably Liara, although I made Ashley into a beast of a soldier.

Grunt and Jacob. I bring them almost every where and good in all situations. Tanks with inferno ammo. That's all my infiltrator needs. Two squad members that can survive on their own while I'm cloaked.

In ME2 Miranda [Warp, Overload AND passive weapon damage bonus to entire squad] overkill.
In ME1 Wrex [with Warp, Fitness and Barrier] he was such a beast he rarely died.

Kasumi. The girl is almost always in my squad. And I'm quite practical when it comes to choosing who to take with me.

Miranda is the best all round squad mate without a doubt, useful on every mission. Then it's really down to how you play, I tend to play biotic heavy - Shepard as an Adept, with Miranda, and then one of Samara, Jack, or, if not filling with biotics, Garrus or Thane) - heavy biotics with upgraded Lift and Throw makes dealing with the collector ship and husks a lot less frantic.
For ME1, I've not really decided, although the Wrextank is always a good option.

Stasis doesn't work on Harbinger....have tried it a few times...

I think Tali's drone is the most misunderstood power in the whole game. I usually play as soldier and that drone always gives me time to do some real damage to tough enemies. When you use the drone, enemy always get distracted.

ME1 - Liara and Kaidan (Biotic juggle!!!!)
ME2 - Mordin and Miranda (Seemed to be more necessary to be able to fry barriers and armors over throwing them around)
