I prefer to mix it up to keep it entertaining. It also feels weird to me for some reason, when I recruit someone and they sit on the normandy the whole time. Still, there are characters I use more than others usually leaves Zaeed, Jacob and Mordin(even though I love him!) fighting hte least.
Are you the same way? Or do you always pick the best for your class or mission?
Do you use all of your squadmates?

For my vanguard manshep; Kasumi and Thane
For my infiltrator manshep;Jack and Garrus
For my adept femshep; Tali and Samara
For my soldier femshep; Miranda and Grunt

Neither. I pick my two favorite squaddies from a storyline perspective, Garrus (bro) and Liara (LI) in Mass Effect 1. In Mass Effect 2 I take Garrus and Miranda because she has to do with the plot with Cerberus and such. Mass Effect 3 is going to be back to Garrus and Liara so yay! Surprisingly, this team is good from a gameplay perspective too, since i am a Sentinel.

Only in Insanity I choose the appropriate members, other than that I take Team Dextro everywhere Shepard goes.

I've recently mixed it up to hear everyone's dialog, but I usually drag Thane and Garrus everywhere, with Legion, Grunt and Jack as alternates.
At the moment, it looks like I'll be taking Garrus and Vega everywhere in ME3. Triple Widow action? Be still, my heart.

I used to mix and match....experienced them all....now just stick to who I like best, most paramount being combat synergy, also for favorite characters....I am lucky that the majority of those I have combat synergy with are also favorite NPC's.....Majority Snipers teamed with my Sniping Infiltrator and Kasumi as my free roaming CQC...works wonderfully for me.
Favorite teams (Insanity included)....Full Sniper Team: Thane and Garrus....Mixed Team: Kasumi with either Thane or Garrus....I don't use Zaeed on Insanity....Enemy Synthetics: Legion and Tali.....and throw Legion into my Sniper mix, once I have him.....I still haven't found a second CQC squaddie that works well for me on Insanity though.

Most to least used (in groups all roughly equal)
Miranda (universally the most versatile character for any class who is with you from the start)
Jacob (mainly when playing Sentinel, Engineer and Adept)
Kasumi
Zaeed (Took me quite a while to figure out this guy is actually so good)
Garrus
Mordin
Thane (Collector missions only)
Grunt (With Adept only would be a lot higher up the list)
Samara (late to recruit and prettty bad without loylaty/Reave, pretty good on the iff and last collector mission though)
Tali (Legions loylaty/overlord are the only missions she is really that good on but even then Garrus is probably better and on most classes only room for one)
Jack
Legion
Morinth
Edit: The order of recruitment is vital and also how early their loyalty missions trigger. If the order you got the dossiers were random (other than Mordin for story reasons) I think it would have added quite a bit to the game.

I never used Kaidan in ME1, either.

Jeez Jack is getting barely any love. I use her alot because she surprises me sometimes. Other times she gets knocked out quite a bit but sometimes she flanks at the best times. I also like shockwave on a npc.

At first, I used Kasumi often because I had her loyalty in the beginning. I didn't even realize how weak my characters were until I got Mordin and all the upgrades. But in my 2nd and 3rd playthrus, it's mostly pairs of Garrus, Miranda or Grunt (I love the Overload and I have success sending Grunt out in the middle of the action).

I don't play on insanity, so I'm free to pick the squadmate I want to have along for the mission without any consideration who might be better on the specific mission.
I do mix them, of course, but not all of them. Tali, Kasumi, Miranda are most often around, though. However, Grunt, Zaeed and Jacob never see any action besides their loyalty mission. The other ones not mentioned are used once in a while to mix things up a bit.

I don't use everybody, no.
With that said, though in ME2 there were so many characters I actually wanted to use I did deviate from how I usually pick my party members, and also I didn't really stick with one party throughout.
The ones I absolutely did not touch outside of needing them for their loyalty missions, or because they had an effect on Tali's mission:
Zaeed
Thane
Jacob
Legion
Stuck in limbo:
Garrus
Mordin (I only really used him on Insanity because I thought he might help which he didn't)
The rest, in order of most to least used:
Morinth
Kasumi
Tali
Jack
Miranda
Grunt

no. On insanity I "have" to use miranda because of how the game works usually followed by Tali
on lower difficulties I stick with my favorites Garrus and Tali
for husk levels I substitute tali with Jack
for uber geth levels I sustitute Garrus for Legion

I tend to mix it up on the first/canon run to try and get a feel for every character's fighting style and personality (and to see who has what lines on which missions) but in general my team is Shepard, Garrus and someone else (usually Mordin on Insanity). I didn't feel like I really had to "balance" my team out, not even on Insanity, so long as I knew what I was doing with whatever class my Shepard was. And in ME1, I always went with the characters I liked to have around more than who was better suited for any particular mission (though Tali usually got brought for geth-related stuff and Liara was brought for Prothean-related stuff).
It's a shame that there are just too many squad-mates in ME2 to go around, there are certainly some characters that don't see as much as action as the others.

I usually have Miranda always in my squad because of her amazing passive damage and health bonuses for the whole squad (as 'Cerberus Leader').
My second squad member depends on the mission. For missions with lots of synthetics I take Tali or Legion. For missions with lots of Blue Suns I take Zaeed. For missions with lots of Eclipse I take Garrus. For missions with lots of Blood Pack I take Mordin. For missions with lots of husks I take either Samara or Jack.
Anyway, Miranda is always with me because of the bonuses, not because I like her (I don't like her personality at all).

My favorite companions to bring along in ME1 are Wrex, Tali and Liara. In ME2 they are Grunt, Zaeed, and Garrus.
And I'm not a fan of the breather masks...
