At which points do you swap Disks in ME2?

If you dont want smarmy answers like Nezzer gave, you should have specified which platform in the topic, or your initial post, instead of leaving it to chance that someone might pay attention to your sig.

But to answer your question, I think its around the Collector Ship/Illium mark if I recall correctly.

illuim is on disc 2, but you only need to swap discs if you are trying to land there. same thing with some of the N7 missions. You also need to swap back to disc 1 for the Suicide mission.

While I get your point, the simple fact is that only the 360 version swaps discs, that is common knowledge. Pointing you play on PC or PS3 adds nothing and just makes you look like a smartass.
