I was playing as a Archer and I noticed that every once in awhile my Quiver disappears. The only way to make it appear would be to unequip my bow and equip it again. Is there a specific reason why this happens? Or is it a bug or glitch?

Probably a bug or glitch, and I'm guessing it was probably caused by one of the patches. Both seemed to add some strange little random quirks.
Just wait until you go to your companion selection menu, and Sebastian is only a head and a bow. No hair, no body, no armor. That's awesome. Hehe, that only happened to me once, but it was bizarre. Easily fixed with a reboot of the 360.
