The first thing that popped into my head was the series finale to Six feet Under.

This video clip is a bit long (8min) but the tear-inducing stuff starts at the 2:50 mark. Once you see how the lives of the characters will play out...it still makes me tear up.

That was the 3rd saddest TV ending I've ever seen. The 2nd was Torchwood season 3 and Blackadder goes Forth.

As for movies in no order Hachiko, Boy in Striped Pyjamas and Saving Private Ryan/Pearl Harbour.
