Picking the wrong character for the wrong role shouldn't just result in their death like in the SM... You get a bad fire team leader he's not the one that should die, one of the ones under his command should (randomize it maybe with weights for loyalty... paragon or renegade nature?).
Or in the writ large conflict... The final battle against the reapers in the skies over earth... Assigning the wrong faction to the wrong part of the battle plan should have disastrous consequences. Stuff like that would work well.
I'm worried that too many important characters are going to die

And that's what happens, if you pick the wrong team leader, your tech dies. And if a team leader is bad, they can certainly die, because they don't have strong tactics. If you wanted them to have a larger impact, they cant go wiping out half your team for one bad call. Two bad calls and only Shepard is left. And I don't mind people dying as long as it isn't forced deaths.

I doubt that you could make it through an invasion from a race of sentient missions whose purpose is to wipe out all sentient life without a few people dying...

I think they mean consequences as in LIME will hate you and LIME2 will leave you I kind of hope not I want the option to pick one of them and patch up the relationship during the game. I don't think the LI war will kill whichever LI you do or don't choose that would be dumb.
