Can't save Miranda in Suicide mission!

1st Fireteam Leader - Jacob
Passed without any casualties!
2nd Specialist - Jack w/ implants
2nd Fireteam Leader - Garrus
Passed without any casualties!
Last stage
My team - Tali and Garrus
Death to Miranda.
How do I keep her alive? I want the achievement for saving everyone! I heard that if I bring someone who isn't loyal to the last boss, they will die, no matter what. So why does she die, when I'm not bringing her?

Personally I'd say just give up, and go back to a previous save where she's loyal. Or start a new career. Either way, I don't think you'll get her to survive this mission. One option is sending her back to protect the crew near the start of the mission.

As far as I'm aware, it's random chance. Basically the system rolls dice to determine if a character dies or not, and their chances of surviving are MUCH higher if they're loyal.
Personally I'd say just give up, and go back to a previous save where she's loyal. Or start a new career. Either way, I don't think you'll get her to survive this mission. One option is sending her back to protect the crew near the start of the mission.

No, it's not random. Not in the least. It's all entirely rule-based. That's what the SM Guide that's stickied is there for: explaining the rules. It also includes a handy chart:

In hold the line, does it mean that if I leave Garrus, Zaaed or grunt, the chance of survival is higher? I always brought Tali and Garrus with me, so does that mean I should bring the people who are on the lowest part of the "Hold the Line" part of the chart?

You need two loyal "heavy hitters" (two of Grunt, Zaeed, Garrus) to protect four loyal "squishies" (Tali, Jack, Kasumi, Mordin). If you take Tali with you to the final boss, you have one less squishy to protect, and unloyal Miranda counts like a loyal squishie. Which means that nothing changes and you need two of loyal (Grunt, Garrus, Zaeed) to protect them all.
So change your final squad like this:
(a) Leave Garrus behind to hold the line
(
BTW: Who did you send back as crew escort?

Simple get your paragon points up. if you dont have enough after the clash between Miranda and jack that if you took jack side which I did. I went around talking to squad members and my paragon points went up. And then I went back to Miranda i get the dialogue a better woman i pick it and now she back loyal. Saved before talking to your squad members because. you might get renegade points. Sometimes you will have to disageree with some of the things your squad members say so you can gain paragon points it took me ahort time to figure it out.

No. For one, the bar probably wasn't actually full. Just so close to it the difference couldn't be discerned. Two, the P/R system isn't just based on the raw number of points you have. The game doesn't have fixed requirements for those dialogue options. Instead it operates on a percentage basis. What it does is look at the maximum possible P/R points you could have at that time, and multiplies that by a constant (e.g. 0.1 or 0.8). If your current total is less than the calculated target, you can't pick that option on the dialogue wheel.
