Been working my way through the Deep Roads and I reached the section where the Ancient Arcane Horror with the last piece of the Topsider's blade should appear.   Only there was nothing at all in that section.   I even exited and retraced my steps just to make certain I had not missed it somehow. 

It is not really a big deal, since I don't use that particular blade very often -- usually just sell it.

But why the missing piece, I wondered.  I've played through the game several times now and that section of the game has always worked just fine.

Are random glitches a common occurrence with repeat playthroughs?

Never had this problem. Have you clicked on the grave yet? The wiki says that the Ancient will not appear until you have visited the grave 1st.

I'm not 100% certain I did.

This actually happend to me too. I went back to retrace the quest, and you have to "activate" the other parts of the quest first before you are able to fight the Ancient Darkspawn in the tunnels. It was kinda irritating that I had to go back through pretty much the whole Deep Roads again lol.

Yes, that was what I did. I did approach the grave but forgot to click on it.

After I went back and clicked on the grave the quest worked as it should.

Ahh! I was wondering about this. I must have done the same thing. Not going back now, but I'll note it for my next playthrough.
