I demand a scene where some Reapers have a big orgy. Maybe in the losing endgame (after all, the entire point of the Reaper war is so they can make baby Reapers, right?).
Maybe a threeway where the Reapers collectively do the Geth and the humans >.>

Less off-topic: I could see a Shepard/Kelly/??? romance happening. Kelly seems like she'd be open to that sort of thing.

The only way I can see a three-way is in the heat of battle, so take whoever you want into your squad during a mission in ME3 and hope it'll happen. It'd be weird if Jacob discussed with Kaiden about banging my Fem Shep, unless they're into that kind of stuff.....
