I'll soon be ready to start the suicide mission, having completed all the loyalty missions and all the DLCs. I have the loyalty of all the squad members except Zaeed and Jack, and a review of the squad screen confirms this. But when I look at the achievements screen, to my horror most of the loyalty achievements are greyed out, including Miranda, who I've started a romance with. Paradoxically, the 'loyalty of Jack' achievement is lit up, despite the fact that I lost it after siding with Miranda during their 'argument'. I've also noticed that the 'Paramour' achievement is greyed out, despite starting a romance with MIranda.
Have any others encountered this problem? Is this a known issue? If so is there a fix for it? Please help as I'm concerned this could affect my chances in the suicide mission, even though the squad screen says all but two companions are loyal.
My Achievements Look All Wrong!

Have you yet to complete the game?
Loyalty achievements should be granted once you succeeded their loyalty missions (as far as I know, the only ones you can actually fail are Zaeed, Thane, Garrus and Tali, and the latter three of those require active action on your part to actually succeed in failing), so losing their loyalty on a later occasion should not impede with the achievement.
The Paramour achievement is most certainly only granted after you did the deed and not when starting and pursuing the relationship.

I cannot understand why this is necessary. Surely when you complete each companion's loyalty mission the respective achievement should light up. And as I previously stated, I have physically completed all the loyalty missions (as the squad screen confirms), so why haven't the respective achievements lit up?

Well, I do not possess inherent knowledge on how the achievement system works, but I'd like to know how it turns out to be after you've completed the game.
The other way round is less preferable, as in you'd have a bug of sorts. It will help to see what system you are playing on and what version you have.
Additionally, you might want (if you haven't already did so) to open a topic on this in the Technical Support section, you are much more likely to get effective help over there than to rely on mere educated guesses by folks like me.

Btw, you can't fail Garrus' mission, but you can fail Samara's

What is displayed on the achievements screen will have no impact at all on what happens during the suicide mission. You could wipe your profile and lose every achievement and all your squad-mates would still stay loyal. It's an odd problem, but you needn't worry about it affecting your game. The squad status screen is the only indicator of who is and is not loyal.
