During the long walk on the collector base the biotic, Samara in this case, keeps jumping around the stop points when battle starts and then disappears completely.

I tried re-starting the base completely on a different character starting a new character, using a different biotic and nothing works. I keep dying because my biotic shield squaddie has gone back to the start or something. 

I can 'ghost' to her leaving my other companions to die but once the fight is over and I tell her to move she disappears again. Ghost to the end of the level and I get a black screen. In short there's no way to finish this level.

Reinstall the game.

Just reinstalled and it doesn't help.

Any other thoughts?
