If anyone can "solve" the white surrounding on the dratted Icon plt, please let me know! I tore out my hair trying to fix it.
I think I am going to try to make it male wearable as well, as my next go at modeling.
Male chest, here I come!

Most colours in the palette translate pure white (255, 255, 255) in a PLT as shiny chrome in the game. In the case of some PLT objects, like clothing icons and cloaks with transparency enabled, this will make those pixels transparent. Unfortunately, though, this doesn't work with body parts - and PLT Editor (the one that shows the various layers as different high-contrast colours) changes the values of some bright colours. Namely, it turns the very brightest few colours in the Leather 2 layer (bright yellow) to white. It also turns pure white to a colour just slightly darker (254, 254, 254), thus ruining the transparent background of clothing icons.
Pelvis icon in your hak uses that darker white - every armor using this part will have white background. To fix this just replace 254, 254, 254 white with 255, 255, 255 white

If this solves it, I will be very, very happy! Thanks for the suggestion!
Hmmm...I edited the psd in PS, made sure that the white was 255, 255, 255. Saved as plt, inserted into the hak.
Still showing up as a white background in the Icon! Grrrr...pesky thing!
So I edited both plt files (ipf) so that they were transparent - still showing up ingame with a white background!
BTW - I do not use plt editor. At least, not for doing this type of work. I normally use the GIMP, load up a plt (with the plt importer/exporter script), then save it as a psd. Then I open it in PS, edit it (or whatever), then I use the BW plt exporter plugin.

I loaded ipf_pelvis038.plt in GIMP, used color picker to check RGB values of background (254, 254, 254) and bucket tool to fill it with 255, 255, 255 white. Saved and replaced the one in the hak. In game icon background was transparent, but there were few white pixels left. Here is the fixed version:

Maybe PLT plugins you use are not 100% compatible or GIPM PSD export doesn't work correctly.
Edit: Other icons are fine btw. Just ipf_pelvis038.plt needs fixing.

I downloaded your package from the vault (hak + test module) and altered only one file (ipf_pelvis038.plt)
I use your hak with my edited file (you can download it form here: http://www.speedysha...1268/Matron.rar) and in your test module the armor icons look like this:

Is this how it supposed to look, or am I missing something?
Edit: And that's how it will look with belt icon ;p

As you can see in the screen shots there is a gap between torso icon and pelvis icon. I've fixed this (moved torso icon 1px down). Also renumbered belt icon to 017:
