I'm having this same issue. I think it's because I'm not unlocking the rooftop. When I get up there, I see no Rooftop Unlocked message like I did on my first playthrough.
I am at 96% game completion, 96% World Explored. I've been to K's hideout, and finished Slim Couldry's quests. I've done all the Chantry board quests, same with Blackstone irregulars, Interested parties, Mages collective and Crows.
I've looked at lists online, and I've been to all the places. I'm not sure how I don't have 100% world explored, to be honest.

Uhhh....rooftop unlocked message? What are you talking about? I've never seen that even on my first playthrough when I got the traveler achievement as far as I can remember. As soon as the cutscene ended the game autosaved and I had a huge fight on my hands....no large message to cover half the screen popped up.

The notification at the top of the screen. Like when you get a new codex entry, it says New Codex up there. I think it does the same with level ups..
The area unlocked message at Ostagar is Area Unlocked: Ostagar, with the star from the Perfectionist achievement in the middle.

No I know about those I just don't remember ever seeing on on the rooftop. Either way my achievement popped up during the entrance cutscene on my very first playthrough before any such message ever had a chance to be shown so even if there ever was one I don't think it had anything to do with the achievement.

Alright. I've loaded a save from before the Landsmeet, and I'm at 80% world explored. I'm heading back into Orzammar to make sure I didn't miss anything there. I've bumped my % of game complete up to 98%, too.
If I get it after the Archdemon, I'll post a list of the quests I've done. Maybe that will help people who don't have it yet.

I saw that and printed it out and followed it to a tee, still no achievement...
Bioware needs to atleast give us some info about this <_<

Strange thing happened to me today. As I was finishing up my first run on the game, just before entering in the final battle with the Archdemon, I got the achievement. I was like o.O. I guess I was really thorough on my first go 'round.

As far as I know, you have to visit every area and building in the game (DLC excluded) in one playthrough. You have to enter the fade in Redcliffe Castle, you have to side with Harrowmont to enter the proving, and you have to open a chantry in Orzammar and come back to visit it.. You also have to go to the thaig that bhelen would sent you to, just for the sake of unlocking (since you have to side with Harrowmont, this thaig is unlocked when you enter the deep roads). I havent unlocked the trophy yet...

Just an FYI; I picked up the achievement without siding with Harrowmont; However, thinking about the play-through, I believe I talked with both reps though before making a choice who to side with, so that maybe enough(?)

Thanks for trying jshunt01, but I have tried that. I have entered every location, cleared each map, hit every sidequest possible and still nothing. I tried taking both sides in the Bhelen/Harrowmont quests in case that counted - making sure to hit the Proving as well.

I was just very thorough to what i was doing in the game. There are some side-quests that unlock new areas and of course the story-related areas too. I got the achievement right before the final battle in the archdemon in my first playthrough.

CSARDiver...maybe you are just trying to hard
One thing I just noticed and will apologize for, is I initially only glanced quickly through the posts. I was playing the PC version and posts seem to be XBox related. There maybe some differences in achievement due to that (?.) Given the degree of difference in posts though, I'm wondering, maybe, rather than having to truly unlock "ALL" areas, maybe it's tied to very specific areas. (?) Anyway good luck!

Previously I didn't "open" Fort Drakon ( like prisoner ), the Antivan Crows' missions ... the Shale's place, perhaps a location in Denerim ( real estate from Couldry's missions ) ...

I didn't get this achievement on any of my 5 playthroughs and I was a little pissed so I went back loaded my first character. I was going through different towns looking for some area I might have missed and I realized I did! On every playthrough I somehow missed the Tavern in Redcliff! I go through, get another % point and finish the game again and I get Traveler on the rooftop this time.

I got this on my my fourth gameplay with only 97% of the world explored and at the same time got 100% game completion, all i did was step in every single building, area, etc and got it, another thing did you do the Slim Couldry's crime wave quest, because during it, he gives a new location around the time you go to save the queen

Ermm...stupid question. How are you checking your percentages? I see no options for this, and in achievements there is nothing that says "Percentage of World Explored". I do have something called easily sidetracked, thats the only thing that says something about a percentage...Thanks to anyone that answers this stupid question :/
edit----Oh lord I'm so stupid. I never looked at the Heroic Accomplishments before, nor the STATISTICS. Stupid stupid...

The crimewave quests are not required. I got the achievement without on my first playthrough.
