Well, Baldur's Gate 1 is the first game I ever played. So I'll pick that one for nostalgia's sake.

Sonic The Hedgehog 2. Great graphics for the time, Excellent music and it;s just great fun to play and has a variety of levels and boss fights. It's everything a great game should be fun to play and infinately enjoyable. I've not once found a game since that was as fun as Sonic 2 for me and i still play it to chill out with. I think a lot of developers nowadays forget the simple mechanics that a game should be enjoyable to play. Sonic 2 was never about being cheap or frustrating the gamer challange was offered in increments getting harder with each stage and boss fight and when you died it was because you screwed up not because the developers had to be cheap to beat you.

Mine is The Legend of Zelda: Ocarina of Time. I never had a video game captivate me again until the original Mass Effect. OoT is, imo, the best game EVER.
The Elder Scrolls V: Skyrim has a chance to dethrone it, though.

I must confess nothing has amazed me quite as much as Monkey Island 2 so far.

Graphics are as good as it gets for 320x200, the sense of fun and freedom it offers is immense, especially for a puzzle-based game, dialogue is hilarious yet there's a creepy atmosphere due to Peter McConnell's awesome music... Grim Fandango is even better as far as art and music are concerned (and it has an extra emotional dimension Monkey Island doesn't have), but when it comes to gameplay it's slightly less perfect. Love the first Sam & Max too... So yeah, I'm kind of stuck in the 90s.
Potentially though, I still believe my favourite genre is RPG, Dragon Age : Origins being my favourite one, but it's too ambitious a game not to have flaws. As they say, perfection is not when there's nothing to add, it's when there's noting to take away. In that regard, I'll stick to the ancient-a** games I named above.

Frontier Elite 2, old perhaps but I don't know any other games that managed to fit a fully formed galaxy with millions of star systems on one humble floppy disk. Awesome achievement considering it came out in the early 90s

KOTOR is my favourite game of all time, I've replayed it so many times and it always find it refreshing. First game I picked up from Bioware too. There's something just so epic about Revan and the relationships he/she forms throughout the game, they just feel really well done in my opinion.
If I had to make a list though it would go:

lol no chance.... I can't do it.. No matter how hard I try, I can't narrow it down to one title and I did try to do so; so much so that if tried harder my head would explode.
What I can do is limit it to system but thats best I can do. Titles that could possibly be greatest (list of exact titles too long as would be talking around 30-40 titles maybe even more) would be back either on~
PS1/PS2 (Suikoden1 -5, Final Fantasy 7, X-com Enemy Unknown, Star Ocean: Till the End of Time, Legacy of Kain: Blood Omen, this list could be as long as 20-30 titles if carry on...)
Snes/Genesis (Phantasy Star 4, Shining Force 1-2, Buck Rogers: Countdown to Doomsday, Secret of Mana, Front Mission and this list again could be over 20+ long if carried on).
Amstrad/Commodore (Elite, Time and Magic, Law of the West, Flashback).
Gameboy (Zelda: Link to the Past and Awakening).
N64 (Goldeneye, Zelda: Ocarina of Time).
Many other systems including PC but impossible to list titles as really are too many that could be best.
I can't even narrow it down passed that system list either, thats how hard it is to pick the 'greatest' out of 30+ years of gaming on more systems than have fingers and toes.. You just asking too much from little old moi. Ultima VII would be my top choice.
Either that, or the first STALKER. It wasn't the most polished or complete experience at launch, but there are so many parts of that game that I love that the whole really is greater than the sum of the parts.

I had several other games in mind but...since this fine man just mentioned Tribes 2, I'll...go with that. I played that game like crazy and few things in my video game experiences were more fun than a light armor sniper kill, or skiing in light armor into a base filled with enemies and dropping a satchel charge explosive and getting multiple kills. Tribes 2 was truely ahead of it's time and was one of the better multiplayer experiences, pioneering many multiplayer FPS concepts. I am eagerly awaiting the new Tribes game in development and hope the new team doesn't decide to screw it up too much.
