I have the latest patch, but on the second time through a certain place on the sloth fade, as a rat, I always get thrown out of the game.  This happens every time I become a rat and cross the same position...just before a rat hole I need to go through.  It just doesn't make sense, because I as a rat went through this position earlier in the sloth fade.  The game is over, and I really don't know why.
