When I read that...... I thought, who better than expert scientist.
I lost Mordin, and only him, for alternate team I had do doubt trusting Garrus and a biotic Samara was clear for me.

Sent Thane to his death by escorting the crew or what was left of it really. I saw Kelly become soup and was disappointed that I would now have to feed my own fish. At the time I thought it was another Virmire choice so who else but the dying assassin looking for retribution. I became frustrated with his loyalty mission due to standing right on top of Kolyat's target and wasn't prompted with Update Thane. Needless to say I moved on.
Picked an unloyal Jack for bubble. Sided with Miranda in spite of Jack not because I hated her, but rather that she soaked up my medi-gel like it was going out of style. Sure Miranda will step out of cover in attempt to stare a YMIR mech to death sometimes, but Jack is taking a dirt nap anytime I take her off the ship. Anyway Kasumi gobbled by swarm, said **** that, and reloaded to send Zaeed unknowingly to his death thinking that the less formidable combatant would die. In this case my other was Miranda. I then thought maybe Zaeed's lack of biotics made him easier to detect. Reluctant to get to the end I didn't reload again
Jack died holding the line. Strangely enough had I left Zaeed alive or not taken Garrus my score for survival would have saved her. She was the one who updated before the baby Reaper. I saw Mordin get blown over cover and seized up for a second really wondering how Jack could be alive and now I had lost Mordin too. That thought was short lived.

Second playthrough, I got Zaead to be loyal via Paragon choice, I did "The Arrival" before the Omega IV relay, plus everything I listed before.
Now depending on what events & decisions Bioware decides to omitted/ignored in ME3, I may play it a third time.

This is exactly what happened to me on my 1st playthrough. I send Zaeed with the crew & I took Garrus with me on the last boss fight => Mordin died. I had all upgrades too. But as soon as I saw that, I reloaded a save & took Mordin with me on the last fight & all survivedluk4s3d wrote...
My first playthrough cost me Mordin.
I figured the crew would need a combat specialist to help them back to the Normandy, so i sent Zaeed.

I have other Sheps that I play differently, but my original playthrough is the first one I will import into ME3....with those squad members dead. I will not go back and correct my mistakes in my original playthrough. That is my canon Shep and in his story Grunt and Mordin did not survive. I do not believe in going back and correcting my original, canon playthrough. That takes the intensity out of the game in my opinion and does not make the decisions seem as important if you're just going to reload and make the "right" choice. That is why I've got 3-4 different Shepards...so I can explore all possibilities while preserving my canon story.

Actually, if you paid a little more attention to Mordin's specialities (which adimittedly takes a sharp eye and ear your first time around) you'd have caught him saying how his area of expertise lay more in the Biological field and not the "technical". He's a "professor". Not an "engineer". His title alone gives it away that he was ill suited for the task

What about all of you? Have you ever loaded your OLD game to see how you did when no information was abundant? It made me chuckle a bit and figured I'd see if others noticed this too (if they are old timers with this game such as myself).
Even going in blind there is absolutely no excuse for anyone to lose a single squadmate by accident.

No, you're not "cool" if you didn't lose a squadmate. You're at least sub-average.
On the otherhand, if you lost a squadmate, barring Mordin (see below), you are a complete freaking moron who clearly cannot read.
It IS easy. Mindnumbingly so.
Perhaps it was so easy that it did, in fact, numb people's mind and that's why they couldn't figure out that the two people you recruit specicically for their biotics were the best choice for a task involving (duh!) biotics. I guess the flying bit or the strongest human biotic title didn't measure up to the glowing qualfication of " biggest badonkadonk". And the mindnumbing obviousness of it all must have also numbed some brains so that a quarian prodogy or a freaking self aware mobile super computer wouldn't seem like the best option to hack something. Perhaps washboard abs or merely having any type of tech power makes someone an expert hacker? And lets look at some examples:
Tali - "Mechanical genius even among the advanced enigineers of the quarians"
Legion - "unmatched technical expertise of a sentient machine"
Mordin - "his true genious is for all matters biological" (Derp! He gotta ommy tool so dat makes em uh gud haker!!!)
I've heard of people losing Mordin at he end, but that is the only unforseeable thing (i.e. you didn't have enough strong people left on the line). I think that can only happen if you, for example, sent Grunt to escort and brought Garrus and Zaeed to the final fight. And only had 10 squadmates (no DLC). Players would not know that Bioware decided to arbitrarily make a "Big 3" tier. On what criteria Zaeed or Garrus was deemed tougher than Samara or Thane, I don't know.
