Miranda. Telling Shepard about how she wanted to stick a control chip into his/her brain, and then saying the only reason she didn't was because Illusive Man told her not to, was a mite tactless. It eliminated any possibility of her gaining my Sheps' trust. I was a little peeved there wasn't an option to bring it up again.

It's not that he "dislikes" Shepard, it's that he doesn't worship him/her as the center of the universe like the kiss ass characters like Anderscum do.

I think Shepherd himself is quite a annoying character, however if i had to say who's annoying Jack, i think she is bit over the top, and seems too unrealistic to me which is what i find annoying about her.

Well Miranda irritates me but I don't know if annoying is the right word for her. Then there are characters that are fun to hate like Udina and the Turian Councilor but I don't think annoying is the right word for them either because while they are hateable they are also highly entertaining.

I guess in ME 1 I can say I am annoyed by Michael and Rebekah (the two people arguing about gene therapy).  I usualy end up solving their little dispute just to shut them up.
