Some answers from the ME3 panel on GamesCom which I attended. They tried to record the session but there were technical difficulties, so there's no footage (there was some in very low quality, but I can't find it anymore):
*Mission complete screens will be gone
*Paragon decisions will not always yield the best outcomes
*Difficulty: while "Veteran is the new Normal", the Casual setting will remain about the same, except that the new enemies may require different tactics.
*Miranda will have a large role in Mass Effect 3
*We will meet possible squadmates during the story, and it's possible that they leave us again (a little vague on the latter).
I didn't take notes because I assumed that everything would be recorded, but some did. Perhaps we'll see more of the answers posted. No questions about other individual squadmates were asked.
They also showed an extended version of the Fall of Earth demo with some cutscenes added, including a conversation with Anderson and the Normandy flying off Earth while the Reapers tear everything to shreds. Very emotional, very impressive.

He said there are decisions you can make which will make the situation in the war considerably worse. He wriggled out of answering directly if we can actually lose the war, but there will definitely be large-scale bad consequences for both Paragon and Renegade decisions.Also really hoping that the Paragon/Renegade choice isn't an automatic win. Miranda plays an essential part in ME2, and depending on the choices you made it seem to reason she would have an important role in ME3. Is she going to be aligned with Cerberus? Will she remain loyal to Shepard if you win her loyalty and/or romanced her? What about her sister?

That scene you just described was shown privately to the press during E3. I’ve read articles describing it in different magazines. I’ve been trying to find it but apparently it was never shown for the general public until now. And still it wasn’t broadcasted.
Isn’t today the final day of GamesCom? …my hopes of seeing something new are fading fast.

I have been there on Thursday for the whole day, yesterday evening for the Q&A and today for the closed presentation and Q&A. And yes, the place was absolutely packed. I was fortunate already had a ticket for all four days because no one who didn't already have one was let in. Even with a ticket, I had to wait about 75 minutes until enough people had left so they could let the next batch of people in. Crazy, I tell you.
