Both O'Keer's and Saren's operations were uncovered. That's simply a matter of logistics. You can't conceal such massive movements of resourses. Of course, the game can say that Cerberus is just that good (especially given the new and friendly Shadow Broker), but there remains a canonic fact that in between ME 2 and 3 Cerberus took serious damage (in Retribution), from which it had to recover. Which, realistically leaves only the option that TIM has had some kind of a "mobilization plan" all the while, and that plan doesn't require a massive investment to be set in motion.
