A short while ago, I decided to finally tackle the challenge this achievement posed. I loaded a pre-existing career and found that the difficulty was set to "Hardcore." In order to get the achievement, does the difficulty have to be on Insanity from the start or can I still get by with changing it at the first opportunity (after the Kaidan-Joker conversation)?

Yes, as long as you change the setting before you activate the beacon on Eden Prime. Oh, if you have Pinnacle Station, unless you're looking for an exercise in frustration, I'd skip it.

I also have a question. When using the Mako, could I still gain full points by damaging the enemy with the main canon, getting off, and finishing them off with a couple of shots from my guns?
