I'm still on my 1st ever playthrough of ME2 and I chose Miranda as Shepard's LI, I romanced her and got the achievement. Then I installed LotSB and Liara spoke about me saving the galaxy because of Miranda, (this shows that it wasn't this DLC that caused the glitch). 
After completing Arrival, I returned to the Normandy to find that Miranda won't talk to me and that Kelly has become the LI. I don't think this is a major problem but it'll affect some stuff in ME3... 

Why did the LI randomly switch from Miranda to Kelly? 

And can I fix it?

AFAIK, Kelly is not a "full" romance. You can call her up in addition to your "real" LI.
Are you sure you didn't accidentally click the wrong dialogue option with Miri at some point? It is an easy mistake to make and I don't know of another way the romance could have just ended.
Maybe you can fix it with Gibbets savegame editor (I'd guess romances are flags like everything else). I never used it though, so I can't really tell you for sure.

Savegame editor? I'm playing this on the Xbox so I'm not sure it'll work.

Actually you can mod xbox360 saves, but I don't know of a way to mod the ME2 romance. The plot flags in Gibbed's only are for ME1 stuff. The only thing I can think of is to use a previous save.
