I have had this problem on every character ive played to this point. All the rubble but one work for me, the one across the bridge has loot instead of being a codex entry and when i loot it it is not longer able to be interacted with, i have DA:O Ultimate and i have done a disk install which did this and i have installed the digital DL of the game and it is still doing it. Also i have both tried it after leaving the area and before i have left the area so its not that bug.

I know where they all are, i just have one that i can not get it had loot instead of a codex entry, the rubble across the bridge behind the rock pillar.

There are two piles of rubble in that area. You are missing the one for the Drifter's cache. Again, see the walkthrough on Dragon Age wiki.

You must find all four piles of rubble. Details on their locations here.
If indeed you have found all four of the correct locations and looted them, I would suspect that a mod is causing the quest to not advance properly. Of the myriad of known bugs, this is not among them; and as you have experienced this on two different versions of the game, it is not your install.
