Party Glitch - Have to Redo Parts of Quests

I often have trouble with this happening, and it is quite annoying as I have to load old saves and replay what I have just done. The issue I'm having is that when I get to points in quests or even go into my house, two or all of the members of my party stay in the party. When I go into my house, I have to leave, remove all members of my party, and then go back in instead of the game automatically doing that. In quests, I have to go back and redo stuff because it won't let me add a certain member to the party. For example, during one of the quests where you need Merrill, I traveled to Sundermount and noticed one of my party members was missing, so I had to come back to Sundermount by myself so the game would put Merrill into my party. Is this happening to just me or is anyone else having this issue?

I also recently learned that I can't complete the final quest (The Last Straw) due to this glitch

I haven't seen this in 10 playthroughs. That sounds like no fun at all. 
When this happens, have you been playing for a while? I know that my xbox had memory issues that would cause blips like that, especially if I had been playing for an hour or more (which was always). You can try clearing your system cache, but past that, maybe someone else has a suggestion?

It happens at certain spots. It doesn't really matter how long I play for. Pretty much any time that my party needs to be changed or go away, it doesn't happen unless I make it do that

Here is the How to clear your Xbox 360 system cache link.
It looks like it may hose your downloaded game updates, and will prompt you to redownload those. I'm crossing my fingers that it works for you!
