The noble character will be discredited by a lie and will have to regain honor
the dwarf character will be a good drinker, and a bit grouchy.

To be fair, that mechanic on the Witcher usually dealt with more discrete bodily components such as organs. Some knowledge when skinning a monster is probably required.

You will die many times from events and circumstances that you had absolutely no way of knowing were about to happen but, thanks to the almighty quicksave, can avoid now that you have foreknowledge.

The female romance-able NPC's will be emotionally aloof and standoffish until you express sympathy for their problems.
The Brutal Killer NPC will viciously murder someone who has surrendered/is harmless right in front of the PC, and the worst comment the PC can make will be "don't do that again!!!"
NPC's will complain about each other to the PC instead of talking out their differences like adults.
One of your NPC buddies will form an irrational attachment to one or more obnoxious ****s in the game and you will be obligated to put up with them/help them out to keep that NPC happy.
If there are vague and inconclusive rumors about something, that something is 100% true and accurate.
If someone asserts something with certainty, it will turn out to be at least 75% incorrect.
Someone, somewhere will be fanatically worshiping a completely inappropriate object that you will be obligated to take away from them. This will not please them.
At some point, lying about something will turn out to be better than telling the truth.
Something important will be stolen from the PC or an important NPC and the PC will be obligated to retrieve it.
Someone will be stricken by a previously unheard-of illness that requires extreme measures to cure.
The final boss will not be the toughest fight in the game.

Female PCs are allowed a scar, sometimes, but only one. ...and somehow noone ever notices it anyway.
