It'd be cool if they were a sort of pseudo romance. You sort of pine for him/her throughout the entire game, but because they're not a romance it doesn't go anywhere. You don't romance anyone else and then when he/she dies you can share one heartfelt kiss.

What if perhaps you start off the game already in a romance with someone. For example, you have a temp companion and based on your decisions from the outset (maybe in the background options or through interaction with the companion) you can decide if you're just friends with said person, or in a relationship with them. Eventually you get separated from him/her and it's presumed that he/she is dead. So then obviously you meet all the new companions and start romancing one of them (if you want) , but later in the game you recieve a letter with no name signed from someone wanting to meet with you urgently and on your arrival you discover that it's your old flame who's tracked you down, he/she also knows about your new romance (if you have one) and it throws a spanner into the works, he/she also needs your help with something. You then have to do a quest for them and if you bring your LI along it makes things even more awkward. Finally you have to decide what to do about your previous love interest and whether or not to break things off with your current LI etc.
You mean sort of like Fable 3? I mean who says we need to have said character as a companion right away? Nadia Grell is a companion in SWTOR for the Jedi Consular and she's basically with you the ENTIRE game before she actually becomes a companion.
They romance you in your background and then "die" but before the end of the game maybe a third of the way in, shortly past when romances are available you get the letter you talked about and then it flows that way
Yeah just like that! Like maybe before we become the Inquisitor he/she does a couple of small quests with you and if you do decide that you're in a romance with him/her (either via conversation or in the background option) then he/she would refer to you as love or say the odd cute thing to you during said quests etc. (Kinda like how Aveline and Wesley are together in the DA2 opening) and then the "big bad" thing happens, which is where you get separated, both of you presume the other is dead and then you're assigned as the Inquisitor and the real story begins. Of course, your lover discovers you're still alive and now the Inquisitor so he/she is trying to contact you, which is when you finally get the letter around midway into the game.
