My theory is that whoever has wrote him (be it as just a npc/cameo or companion/LI) decided to give him a second name (for whatever reason) because he's no longer going to be addressed as "Knight Captain Cullen" or "Templar Cullen" because title and rank will pretty much be obsolete (especially if he is a companion). So rather than him just introducing himself as "Cullen" they decided to Jazz it up a little by adding in a Surname, that possibly could be part of his back story.
Good theory!! We've had single-named characters before ("My name is Fenris") so having just one name wouldn't normally be an issue. But apparently Cullen DOES have a second name, indicating that he would fall back on this second name now that he's not longer Knight-Captain.
Yeah, I like to imagine that he used his rank and title as a bit of a shield to avoid being too over familiar or warm towards people. Introducing himself as "Knight Captain Cullen" sounds a lot more formal than just "Cullen". But, take away his title and what does he have? Just "Cullen", just himself. He's no longer a Templar, but a man with no rank to fall back on. So what does he do? Introduce his Surname into his introductions instead, thus still maintaining a formal approach, and a way to remain detached and seemingly prevent himself from getting attached to people.
I'm half expecting him to introduce himself to the Inquisitor as follows:
"I am Knight Capt... uh
*pinches bridge of nose* Cullen
*Insert Surname*"
