Like, every now and then he'll forget his name or display weird behaviour and eventually he'd tell you that the years of lyrium abuse is starting to taking its toll. Perhaps he'd admit it after a certain point in the romance (before things have been consummated) and he'd give you the option to end the romance because the protagonist would only eventually end up nursing him like the elderly, which he doesn't want.
Then you can either end it, or be like "No, I won't lose you! I want to cherish every waking moment we have together... I'd rather spend the short time i have with you, than never have you at all." *Que violins and sexy time saxaphones*

That could be his companion quest. You could speak with him a few times leading up to this, and question if he's feeling all right (because your PC notices he's talking strange, here and there). He brushes if off and says he's fine. But finally things come to a head, and you have to figure out a way to cure his addiction. Begin quest!

Honestly, I don't have anything prepared for 1800. End of the semester was brutal, and then the day after grades were due, I got hit by a terrible cold. Still dealing with it... so I don't see myself writing anything for the celebrations due to sheer physical misery *sigh*
