Actually, the World of Thedas talks about a lot of different important families in Thedas by name. Could be we'll be playing as a member of one of those. My guess is we'll definitely meet at least some, as quest-givers at least.
If we're called by our last names like we were in DAII, we probably won't have different families as backgrounds. But there are still plenty of reasons that you could be a Pentaghast (just as an example) and one would be a Circle mage, one would be a street urchin rogue that was separated from their family as a child, one would be a skilled warrior who had the best training their family upbringing had to offer, etc.
I'd presume that it'll be one Surname for all backgrounds (again, like Shepard). Whatever the Surname is, it'll have to have a good ring to it and sound good when being addressed by it constantly. Hawke and Shepard are prime examples. Pentaghast just doesn't role off the tongue in the same way.
