LOL! He probably deserved it!
Being called by your last name seems a lot more normal for a guy character. The only people I could imagine calling me by my last name are:
1.) High school girls who hate me
2.) High school PE teachers
Of course, I share my maiden name with this guy:

So I usually got called by my first and last name smooshed together just because people thought it was fun to say.

But... but I really like being a girl. I don't want to be treated like a guy.
I realize that's just one opinion of many, though, but I think most people would probably have to agree that it was weird going through the whole entire game (weirder because it took place over the period of a decade) with all your friends only calling you by your last name or "Champion."
The "Warden" thing didn't seem quite as weird since it was only a year's duration and "Warden" was such an important title. Plus most NPC's you spoke to didn't know anything else about you. By DAII's end, I was really hoping to be more than just "Hawke."
Being called "Commander" in DA:A made even more sense since military titles are used often in society instead of "Mr. Lastname" or "Mrs. Lastname." Even then, with close friends....
I don't know. I don't have a good solution to the problem -- I just know that I thought it could be better. And as DG says, we fans have the luxury of being able to complain without having to think about solutions.
