Does make we wonder what Cole would be like as a companion. I'd feel less creeped out if the victimized child persona were replaced with his self-aware 'demon' one.

Yes but in my super special ultra romantic version of the tale, my PC is the one person who doesn't induce stabby inclinations in Cole. Actually, to be honest, so much was said about Cole prior to reading the novel that I expected him to murder people right and left, like, tap Cole on the shoulder, and OOPS, YOU'VE BEEN MURDERED! But his killings are actually quite nicely explained. They are, in a sense, mercy killings. And he voluntarily stops. The only killing that 100% murder is Lambert.
I also didn't get "child-like" from Cole, but more "traumatized victim." I think he went to the Circle at around 12. Cole the Boy might have died then, or merged with the demon/spirit (whatever), and has lived at the Circle for the past 10 years (because Cole said to be about 20). (He was about 12 in his dream in the Fade)
It could be however that he never progressed beyond 12, which would indeed be child-like.
And yeah, I'd have to not get any "mentally delayed" feelings from him in a game if he were a hypothetical LI, of course.
