I guess it could've been on record about what happened to Meredith? After all, the Seekers are privy to Templar records etc since they're the ones who investigate the Templars when required.
Late to the party as usual and saw this while catching up, so I wanted to add my two cents~
It's obvious that Cassandra believed Hawke planned the whole bloody thing, so I imagine that includes Meredith's madness. Is Meredith to blame because of the red lyrium and how it drove her paranoia to complete insanity and curropted her want to protect people and turned into a hunger for power? Possibly not from where Cassandra was standing at the beginning of the tale. Instead Meredith was another of Hawke's victims and someone Hawke plotted against from the very beginning.
Just like Varric says, Hawke isn't innocent per se, thanks to how he gets caught up in everything that happened and was right in the middle of it all, but he isn't some mastermind who plotted the downfall of the Chantry, Meredith and everything else like Cassandra thought. Instead the poor sod was trying to look after his family and survive and in the end he only succeeded at the latter.
...And possibly partly succeeded at the former if his sibling is at least alive. Though depending on the circumstances and which sibling it is, they might not consider their lot in life lucky. My Hawke was total bros with Warden!Carver by the end of it, so that sorta worked out aright.
And means my Hawke will lose both his brother and his boy to the calling. (ʘ‿ʘ✿)
