Yes, I don't believe "unstable" was supposed to be his natural state, based on the way his writer talks about him. So, I presumed the same thing. Just because the Warden died in the joining, doesn't mean Uldred didn't do his thing at the Tower.
Regarding his eyes... I always figured the circles under his eyes were because of his PTSD. I imagined it would affect his dreams and such, and so the circles were from lack of sleep and just to show in general that he'd been through a lot. That's an interesting idea about him being deprived of lyrium, but I'd be surprised if that were so. I can't imagine the Order thinking it of any benefit for him not to be addicted and without it he apparently loses his magical immunity, plus is likely to be become unstable. Doesn't seem like a winning combination for the Knight-Captain of Kirkwall if you want him to be high-functioning.
To address Melca's question, I bet he'd go through various stages of anger and denial that the Chantry would use and abuse the templars in that way. If he were still on the lyrium, I could imagine him forcing himself to kick the habit at that point, no matter how painful or horrible it would be.
@ReggarBlane, I like your idea of him turning to his faith to manage his withdrawal. I bet you're right; I also don't see him turning away from his faith in Andraste... even if he loses faith in her church. But, since the Chantry seems to be somewhat "in the right" in this whole mage-templar war, I still see him staying true to the Chantry. He might break up with it for a bit if he found out the lyrium was just a shackle... but I bet he'd still come back to the church in the end.
