I dread to think how many of my followers will end up dead during my first playthrough, LOL!
I will admit that my first playthrough is gonna be a disaster. The first one is always terrible for me in DA games. My main goal however in my first playthroughs is that I try to make sure both my LI and I stay together, and he doesn't end up dead somehow. Whatever else happens, happens. Que sera, sera.

I always end up missing someone. That's why I wanna know who all the companions and important people are ahead of time so that I don't miss any this time around.

First game of DAO, I didn't pick up Leliana, and didn't know who she was until after I completed my first playthrough.
Out of 7 DA2 playthroughs, I have only managed to keep Isabela once, and that was the playthrough I finished last year.
First playthrough of DA:O, I had to sleep with Morrigan because I couldn't convince Alistair

Same! I also missed Isabela the first time in DA2 as well! I've only managed to keep her once, and that was by sleeping with her because I couldn't get her to full friendship for the life of me!
Vivienne's smart. I think she will follow whichever path that leads her to greatest amount of power and influence. If the mages are freed, but she still can hold power, then she'd probably support it. Whatever leaves her on top.

DAO? my first grown up game in a decade, so I shamelessly used walkthroughs for the whole thing. Didn't miss anyone!
DA2 - tried really hard to not metagame, but failed miserably. So didn't miss anyone!
DAI? will probably try not to read ahead, but fail miserably. And not miss anyone!
I've never missed a companion, but that's because I am a dirty cheater and use the guides. 
For Isabela, I metagame the heck out of her, depending on if I want her around in Act 3 or not. (Literally, I have a notebook full of missions and when to take who so I can get the kind of relationships I'm after.)

You know what? Regarding Vivienne, I'm not even channeling Loghain anymore; I'm channeling Harwen Raleigh of all people.

For DAI my husband is going to do for me what I did for him when he played Mass Effect: go through a walkthrough as I play and when I'm about to leave an area / do something irreversible / cause the game to "get on an unstoppable train" for a bit, he'll stop me if I'm missing an important item or conversation. Saves me from spoilers, and I'd already played ME / he's never going to play DA so he doesn't care.
My favorite follower list is unfortunately DAO-top-heavy, and I'm sure if I'd romanced a DA2 person other than Sebastian that would be less the case:

Alright, I've given in. 
I'm only going to list my very favorites, because I would never be able to prioritize and organize all of them. Or rather, I could, but it would end up taking me several hours of minute fiddling, because that's what my brain does. :|
1. Alistair He is one of my favorite fictional characters ever. Seriously, I could go on at great length about the reasons I love this fool as a character, and love his progression through Origins.
2. Anders I don't differentiate between Awakening and 2 (although I do get why people do). He, also, has a tremendous story arc, and I could go on about him as well.
3. Wynne I like her, I think, because I am like her, a bit, and I would put her on the list of fictional characters that I would like to have a beer with. 
4. Isabela I love that, to me, she is just so *real*. She's flawed, and damaged, but strong and capable.
5. Nathaniel Howe (At least today, he's number 5). I can't think of anyone I'd rather see as Commander of the Grey in Ferelden. Also, that bow.
(Now, there are only a couple of characters that I actively don't like, so if someone's not on this list, it's cuz this was my gut reaction, first five. Otherwise, as I said, I'd be here forever weighing options and all. *grin*)
