I think your companions etc noticing you're a blood mage was just too difficult to include in the story and so it gets handwaved.

perhaps not as extreme as that lol, but carver, fenris, aveline hate mages even anders hates the use of blood magic i expected these charaters to look down on me using it.
it gets built up as the most evil of evils in DA2 but their are no repercussions for actually being a blood mage.

Yes, those mentioned are the anti-mage companions of the group, and yet they repeatedly help you protect mages and fight the templars. Even Anders can help you kill off the mages in the end (his motive is a cop-out).
Perhaps Bioware could have any nearby templars to hostile if they see you using blood magic or lose disapproval with companions if they see you using it.
If Bioware did "punish" players for using blood magic, then I wouldn't be surprised to see people complaining on the forums about how they are penalized for using the specialization. It would add to the realism though, as blood mages are typically outcasts, unless they are around other blood mages.
Bioware would really have to plan ahead for this, and if blood mages got all of this attention, then every specialization would need it's own time in the spotlight or blood mages would seem like the "favored child" of the series.

They should at least try to do so, in the name of realism. Not sit there asking if I've seen any dangerous apostates while I'm on my way to help a bunch of apostates escape, as I'm in the middle of stabbing myself and raising the dead in front of him while I'm in the process of getting ambushed by more ninja waves.

In DA:O if you took the Blood Mage Specialization before you completed the Circle Quest, Wynne will not join your party after the quest is over. I think she in fact attacks you because of it or something. I'm not sure because I never played my mage from the circle that way. There are some reactions in DA:O but they aren't party banter based.
Also in DA:O Wynne is the only party member to have such a scene in the game. Morrigan would be all for blood magic if used properly. Alistair is a Grey Warden and as such knows that all kinds of magic are used. Leliana she probably would have something to say but wouldn't put up too much of a fuss. Shale wouldn't give a damned either way. Zevran is a practical sort of companion and wouldn't give a damn. Ohgren is a dwarf, he doesn't give a damned about magic. Sten would probably have some strong opinions of your warden mage being a blood magic user but never got included I guess.
Just some conclusions I've come to. I believe if DA2 had not been rushed there might have been some reactions to a Blood mage Hawke but alas the game was rushed so it wasn't really completed as we all had hoped for.
Still a good game though. As for realism... we don't need realism in everything we play or watch. It can at times take away from the experience in and of itself. Just my opinion.

All I want is for my companions to recognize that I'm a blood mage. I don't care if no one else in the game does, but my companions, who spend so much time with me, should know - even if its only brought up in one conversation the entire game, or if you get +5 to rivalary from everyone except Merrill, who gives you +4 to friendship.
It's the one thing that breaks total immersion for me.

I wouldn't even really care about the +/- to friend or rivalry as long as they acknowledge what your doing. I mean every mage practically in dragon age 2 besides a handful were blood mage's and barely anyone batted an eyelash.

how did you manage to do that? it was alain (if thats the guys name) that revived carver/bethany in my play throughs

I do agree there was way more chance for RP options based on class, but I wuld have rather seen them focus on mages using magic to accomplish things, BM is too specific and only a certain ammount of players would benefit. I think a really great idea would be for different companions to have specific conversations with Hawke about spec choices. BM wuld be a good one, but what about a Bethany freak out because i joined the templars, etc.

Things like shapeshifter, the shapes had to remain in the memory the entire game even if you never used it. I imagine Ranger was the same. So out they go.

The only thing "special" about Hawke in that regard is something called "plot armor".

Yes, this is just laziness. Not to mention the fact that being an ordinary mage has little impact on the story. If you did decide to become a bloodmage, the least they could do is have someone like Fenris confront you.

Laziness?
It is called working within strict limitations. Just for an example if the scene with Fenris and Hawke bloodmage were added another conversation with him most likely would have to be cut. I'm not saying that is good or bad, right or wrong but that is just the way it is.
