 Especially for someone at the end of the game but how can I equip health poultice's to a character so they actually bring them into battle. 

Also what is the healing item or whatever that pretty much everyone has default in one of their slots, Im pretty sure I have so many of those but removed them from some of the characters slots

I am pretty screwed now lol. I know how to equip any of the moves, spells such as heel but some characters have nothing to heal themselves with and its getting me mauled

like when i go to the menu of health poultice i dont see any way of assigning that to characters and I dont even know what the default one they usually all have in a slot is but since a couple of them still have it I know i have more than 40 of those if there was a way to re-equip it.

any help would be great

I play on the PC, so it may be different, but I use the tactics screen to tell the characters when to use health poultices. The setup varies along the course of the game, but it looks something like this:

left side of menu: health: less than 25% (or whatever number you prefer)
right side of menu: use: least powerful health poultice (or whichever strength you prefer)

Access to the tactics screen is through one of the icons up at the top of the screen.

You don't equip poultices. Hit L2 (I think) and pull back on the stick. At the bottom is an option to use one of them.

The alternative is to pull left and select them from the potions menu.

Another is to do like Gandanlin said and set it in the tactics so they do it automatically. I prefer this method.

To assign health p. to a button, I think all you need to do is hover over that "use potion" thing while holding down L2, you know the one at the bottom. Then hit triangle and assign it to whatever key you want.

I usually use poultices from the radial menu, as I'll reserve the hotkeys for abilities that I often use.

The only way to hotkey poultices on consoles is to map the quick heal command to one of your hotkeys as if it was a spell or ability. If you want your companions to always use the strongest or weakest poultice, that has to be set in the options menu.

Even if you don't have poultices mapped to one of your controller buttons, you shouldn't feel "stuck". Do you have a healer mage? When/how often to they cast healing spells? Usually I'll have Wynne cast heal at 75% health, regeneration at 90% health and force field at 50% health. For the most part, my healer mage is all of the healing I need and health poultices stockpile over the coarse of the game. I'll lose more health poultices from the quests that ask for them, than from actually healing my characters. It just helps to have your mage heal early and often while the rest of the party does their best to avoid taking damage in the first place by having high defense and/or a good armor rating, or just avoids enemies alltogether.
