If you've got the ultimate edition play it like this:
Origins with Stone Prisoner, Warden's Keep and Return to Ostagar.
Awakening.
The Golems of Amgarrak.
Witch Hunt.
Then in any order:
Leliana's Song.
Darkspawn Chronicles.
Those last two are completely standalone chapters with their own characters. The ones listed with Origins expand the original campaign. Awakening, Golems and Witch Hunt allow you to import your Origin's character. Once you've finished Witch Hunt you can then finally import your character into DA2.
Hope that helps!

No way! Witch Hunt is a breeze even on nightmare, and can easily be done before Awakening. It also gets you Vestements of the seer, one of the best robes in the game and can be acquired early. Golems of Amgarrak should also be done before Awakening, unless you want to beat everything and only then get the reaper's cudgel, an awesome weapon that sells for nearly 500 sovereigns. I have only ever tried it on nightmare with my level 21 mage (and won) so I dont know if you get the reaper's cudgel if you play on a lower difficulty. If you do, then I recommend beating Golems on easy right after you finish Origins, then beating it on nightmare after Awakenings (unless you want a real challenge!). Golems of Amgarrak on nightmare is extremely difficult on nightmare even on PC and is almost impossible on consoles unless you use a high level imported awakenings character. I personally refuse to level grind and it took me 12 hours worth of tries to beat it on PS3 with my level 21 mage, but this is obviously not recomended. Here is a real list:
-Darkspawn Chronicles
-Leliana's song
-Witch Hunt
(these three first for the gear. Note that Witch Hunt is kinda spoilery so if you care about an in depth experiance over optimal gear save it for after Origins)
-Origins and all Origins in game DLC (Stone Prisoner, Warden's Keep and Return to Ostagar)
-Golems of Amgarrak on easy (maybe Nightmare if you have PC [Bronto Tactic, better controls, better angle control etc.])
-Awakening
-Golems of Amgarrak on Nightmare (if not already done)
-Darkspawn Chronicles

If you want to play one character across all, you have to play WH last. Hints the recommended order. Which odds are most people will want to do.
It's also fun to play DAO and Awakening once without having all the unlocked bonus gear. It also doesn't mess with the overall story and timeline. No spoilers this way.

I also recommend playing without all the Bonus-DLC-items. You get them without any effort after unlocking them and they are slightly more powerful than even the best things you can afford in DAO. Additionally, there are enough good vanilla items in DAO/DAA which could be used when you're not going to use the vastly overpowered ones.

The one in uber's post.
DAO, Awakening, Golems, WH. The last 2 can be played whenever, but might as well save them for last given they add more context and don't fit in neatly.
