I'm trying to complete the achievement you earn by completing the game without the main character dieing. I have made it to the Bracalian Forest, and at the part where the party goes to sleep at the abandoned campsite and when you wake up only one party member is alive. I assumed this would not count against me for this achievement, but when I click on Heroic Acheivements for my character, it now has Injuries = 1. Is this going to screw me out of the achievement?

It does seem that way, i am not sure how it is decided who lives, i have had 2 party members live, both my mage and Wynne lived while Allister and Leliana fed the worms. I could be wrong but that seems to be determined by mental resistance so you may want to reload an earlier save, save that encounter until you have more mental resistance gear or stats and try again. Or you could just avoid the encounter entirely if the achievement means that much to you.

Another question, I thought I saw an acheivement for this somewhere (thought it was called Hardcore), but now I can't find it. Is it indeed an achievement for not letting the hero character fall in battle?

Its determined by willpower.
If you enter the camp and leave without touching anything, none of your characters will die and you'll force the abomination to come out and play. The more items you touch before you try to leave, the more willpower you need to overcome the drowsiness.
And yes theres an achievement for your hero not dying. Its called Kind of a Big Deal.

Got Kind of a Big Deal pretty much by accident on my second, Nightmare playthrough.
I didn't realize I was eligible until I got Indestructible after the Landsmeet, and then I was super stressed the rest of the game trying not to wipe, especially when Wynne immediately died against the Archdemon. ;x Finished him off with only my PC alive, like a ****ing hero.

You can get away from that camp without anyone at all dying. It depends on wich of the initial answers you give when you enter the camp, I think it was the "Get away" type one, then try move away and the demon should spawn and charge at you with nobody dying. I hadn't put a single point into anyones Willpower, so it shouldn't be necessary to pump it for getting the Dusk Ring. A tip would be to quick save before you go near the camp.
Ironically, it still says "as you wake up" etc. after killing the demon, although nobody has actually slept and gotten any injuries, heh. It's one of the few bad dialogue/texts I've found. Another pretty bad one (in my eyes) is when you have killed the Werewolves after they become human, and you enter Brecilian Forrest again, you get a scene with a Dalish that wants to kill them (this happens regardless of what you have done afaik.). One of the options is (Persuade) (Lie) Nowhere. I've killed them. I mean.. what am I lying about there? I actually DID kill them!

Isn't the achievement for not falling in battle, not never receiving an injury?

I managed to glitch my way out of that camp. Use a long range weapon and try to find an enemy you can fire at from within the camp (In my case it was some wolves on a hill on the other side of the water). While in combat the event triggered on leaving the camp never happens (at least it worked pre patch)

that camp dont count as dieing i got the achievement even if i "fell asleep" in this camp
