So when this game first came out I played it 6-7 times (twice with each class for sure) and in all of those I tried to make sure I did things different just to see how things played out. When the last DLC came out I played it a few times though I only used toons that had already finished the game.
With the next DLC right around the corner I decided to play the game from beginning to end just to refresh the story. Playing as a rogue (my favorite) I have had encounters and cut scenes I have never seen before and now has me wondering if more was added to the game with the last DLC. Things I experienced for the first time are:
* Anders casts some spell on the crazy Bartrand which helps clear his mind some. He ends up in a sanatorium.
* The elf girl you save from the guy who kills elf kids cause they are too pretty. There was a cut scene outside Merrill's house with this elf girl thanking Hawke for saving her. She recently joined the guard however was not in uniform.
* Just to the right of the Emile's parents mansion there is a alley behind that mansion. Probably has always been there but I never saw it till now.
* In act 3 on the Wounded Coast map there was a lone female who talked about being saved and now in love with Feynriel. Seems a certain dreamer mage has been naughty.
* Some companion banter that I had written down but can't find now.
There were a few other things that I cannot think of now. It doesn't surprise me that I had missed things before but I am very surprised by how much new stuff I have seen that makes me wonder if at least some of this stuff was added with a patch or DLC or maybe and more likely was fixed with a patch. Or was this stuff always there and I just missed it cause I didn’t have the right person in group to trigger it?

- Anders can obviously only cure Batrand if he is in the team (Perhaps you did not bring him before), he has, however, always been able to do that.
- The elf girl is the result of you killing the magistrates son. (she won't be a guard if you don't kill him, also with Aveline the conversation is like a line longer)
- I missed the hidden alley a lot of times too on my first play throughs, but I don't think it was patched in
- The Fenriel quest is Who needs rescuing which up untill the latest patch was bugged, so yes that is a fix.
- Companion banters are properly a result of you taking a different choice in rivalry/friendship - but one of the patches did tweak them a little, so it might be that which you notice.
- The elf girl is the result of you killing the magistrates son. (she won't be a guard if you don't kill him, also with Aveline the conversation is like a line longer)

I figured most was due to having a certain companion at a certain time plus doing something the right way to get the result. I am just surprised I missed so much before.

The most recent thing I discovered, after completing the 'On the Loose' quest, happened If you let Emile de Launcet lose his virginity before taking him back to the circle.
If you go back to his parent's mansion (at night), you can eavesdrop on a funny convo between his parents & the barmaid about her claiming she was knocked-up.

The Feynriel one is the only case where it was bugged in my version - everything else I got depending on who was with me. I never tried going back to Emile's family after visiting the Hanged Man - I'm going to try that now just to get his mother's reaction! I also did not get the conversation between Fenris and Dog because I didn't have the latest patch, so I may well have to play through again just to see what I have missed as there seemed quite a long list of things it fixed.
So far as companion banter is concerned, I find taking different people on shopping trips or sight seeing round the city is a good way to experience the full range, particularly as I tend to have a core party that I like to use so would otherwise never get to hear a lot of it. When going out of town I do sometimes deliberately mix and match on different games so I get to see those interactions too. I am currently working through the various permutations of Legacy.
So far as companion banter is concerned, I find taking different people on shopping trips or sight seeing round the city is a good way to experience the full range, particularly as I tend to have a core party that I like to use so would otherwise never get to hear a lot of it. I saw the conversation between Emile's parents and his "lady-friend", only because I looked the quest up. That's a funny scene that's easy to miss.
