By chance, I managed to save everyone my very first play through. I did it by completing all gained squad member loyalty missions before going to get the IFF in the first place.
First off between the beginning and Horizon you do five missions counting Freedom's Progress. So focus on getting those dossiers filled. I start with Mordin and Garrus, since they are both on Omega. Get the two of them, then go get Grunt and Jack. Once you've done your fifth mission *note it's not necessarily five mission, Horizon pops after the final of the original dossiers are fill*, TIM sends you to Horizon. After Horizon, TIM gives you three more dossiers and you'll also get Jacob's side quest immediately. At this point, I talk to everyone and do side quests/galaxy hopping. You should be able to get all original team personal quests in this period. I don't start doing any loyalty quests until I get Jack's to pop. I do Pragia first, then Jacob, then I recruit Tali. At that point, I should have talked with Grunt and Mordin enough to get their loyalty quests to pop and I head to Tuchanka. Garrus' quest will also come up, but I always leave his until after I recruit Thane and get his loyalty mission as well, since both are on the Citadel.
Now after five more quests - main mission quests - post Horizon, Joker will tell me TIM wants to chat. After chatting with him, where he tells me about the abandoned Reaper the Turian's supposedly attacked and you automatically go there. At that point, Jacob, Grunt, Mordin and Jack are loyal and I've recruited Tali. After the mission, you'll be back in the comm room talking with everybody. I always, always opt to finish recruiting people before running down the IFF on Mnemosyne.
And that's what I do. I go to Ilium and recruit Thane and Samara and do Miranda's loyalty mission. I played LotSB here the last time I played through as well. I know a lot of people do LotSB after the SM, but I like to play it in game. The bonuses do me more good in game than after I've done everything there is to do. At this point, I should have managed to get Thane and Samara's loyalty mission, so I head to the Citadel and do Thane and Garrus' loyalty missions, leaving Samara's loyalty quest for last. When that's done, I finish galaxy hopping and side-questery until the only thing left to do is the IFF.
Then I go do that. Pick up Legion. I turn Legion on BEFORE anything else - I don't look at the galaxy map at all. I go straight from the Comm room down to the AI Core. Apparently, this is important. I talk him out, getting his side quest in the process. Then I go do Legion's sidequest, while the IFF is being installed but is not yet ready. This is really important too. You can do one, just one, mission/quest between picking up the IFF and hitting the Omega 4. If you don't talk to Legion immediately, his side quest won't pop until after the crew is kidnapped and if you delay going to save the crew at that point, people start to die. After everyone has been taken, you have to go straight away to save them. Period. But at this point, for me, the entire crew is loyal, incl Legion, so it's not an issue.
With everyone loyal, the final mission, Omega 4 is all about picking the right crew for the right purpose. I send Tali through the vents. I leave Garrus or Miranda in charge of the fire teams. I use Samara for the trek through the bug room. I send Mordin back with the crew and I take Thane, Jack, or Tali, with me for the final battle, being certain to leave Garrus and Grunt to hold the line. Everyone is loyal, everyone lives(including the crew). YMMV of course, but I had to actually avoid doing things in the order I played the game the first time through in order to get a 'bad' outcome.
