A few more questions.
You do have the 1.02 patch installed I assume?
Some ATI cards also have an audio device onboard, if yours has one, has it been disabled in the control panel hardware manager?
Do you have any "unknown devices" listed in control panel hardware manager?
What other apps do you have active in the system tray (row of icons next to the clock)

I have 1.02, manually installed it as well incase steam didn't do it right.
ATI sound is disabled in device manager
Hardware devices are fine.
It's not another app that's causing this, have tried to run it with absolutely nothing else except required operating system programs running. (see previous posts)

Game runs fine as long as the window isn't active, so something is wrong with the config for the unreal engine, especially since all processing gets dumped to core 1, including anything that should be on the gpu as soon as the window becomes active. The gpu activity goes to 0 when the window gets focus, when it doesn't it looks like a normal average load of 30-50%, as well as both cores on the processor.

If its running fine through an inactive window, it leads me to believe that there is something in the audio configuration which is causing this issue. Edit your My Documents\\Bioware\\Mass Effect\\Config\\Bioengine.ini file, so the audio settings look like this.

[ISACTAudio.ISACTAudioDevice]
MaxChannels=64
TimeBetweenHWUpdates=15.000000
MinOggVorbisDurationGame=20
MinOggVorbisDurationEditor=4
TotalAvailableChannels=62
DeviceName=
ReserveChannels=2
UseEffectsProcessing=False

Note: If you re-run the config utility, or open teh audio properties in game to adjust volume levels after you make these changes, it will reset the settings to default and you will have to edit it again. If that doesnt work, I'll PM you my email address and send that file to me, and I'll have a wander through it to see what I can find.

Sounds good. I did mess with that section, but I don't remember what I changed the settings to, so I'll give it a look later on. It may very well be audio, when I disable all hardware accelleration from the control panel, the logo screens actually run right until the demiurge logo, then I get audio stutter for a second, and it loads that as well. Weird thing is, I have gotten the logo screens to run fine with hardware accelleration enabled on the sound cards too, it seems kind of random. I'm wondering if it's a problem with Ogg codecs?

I'll report back later.
