That's the thing though. By not taking the risk presented you'd be advancing the plot differently. That is what I'm advocating here. Actual choice which changes the landscape of the conflict. If you want everybody to survive then you should be required to focus on that. I'd love to see actual risk assessment and more emotion from Shepard when he's gambling with lives.

Huh.  That would be pretty interesting if implemented well.

It's a game about a massive war. Emphasis on massive. I don't think it's too out of place to expect some death and carnage to go with it. To me it's a matter off believability. BioWare wants to sell me on the idea of these guys being a huge threat to the universe. They're not going to accomplish that by having Shepard and crew drinking hot coco while leisurely cruising through the game without sacrificing anything and then living happily ever after.

I don't think I asked for that in the OP.  "Survive" does not imply a no sacrife, hot cocoa happily ever after.  It's one ray of sunshine in the darkness.  You want the game to be more realistic, I want it to be enjoyable enough that I don't lose interest in it.  I need positive emotions in there for enjoyment, too.

I would prefer to be able to beat the game with all squadmates alive.  I advocate gains in this area meaning losses in others, so long as they aren't so ridiculous it might as well be a non-choice.  I accept a revisit to the Virmire scenario as a compromise, so long as the presentation is more involved and less blatant.

As for Cheez's statement, I'd not be too big on the return to Virmire. It's been done before in the series, and now I know to expect it. If it shows up again, my reaction will be more "Hah! Caught you reusing plot devices, lazy writers!" instead of "Wait, you want me to decide what?" I suppose I still might accept choosing between two squadmates if they were clever enough about writing the choice that it didn't feel like a complete rehash.

I've a question. Would you accept extremely large losses to keep your squadmates alive if it wasn't immediately apparent that saving your squad would have such a bad effect?  Instead of the choice being "Save a billion people or keep your squadmate," it might be "You'll lose a squadmate if you prevent the Reapers from destroying this shipyard, but later those ships turn out to be the only thing that keeps a billion people from dying."

I'm honestly torn about this one. I don't think I'd mind if there was some hints that something bad might happen, but I can see people complaining that it's still too over the top or the consequences not being clear in the first place wasn't fair.
