I played through the first time, importing from Origins, from my save in the epilogue. I had played through with a mage, had make Alistair king, and he and the Warden were quite alive at the end. I even asked him to "free the Circle of Magi" as my boon. All's well and good. . . I play through DA2 to the end of the second act, Alistair appears, and my husband, looking over my shoulder, said, "Huh, that's odd. In my game, he introduced himself as a king, not as a Gray Warden."
I was pretty far through, obviously, and didn't think much of it until I played through on my rogue, got to the second act. . . Hey. He's still not introducing himself as a king. Checked the Codex, sure enough, it claims that he stepped down in favor of Anora.
Gritted my teeth, imported from a save in Awakenings, just in case, somehow, some setting somewhere in Origins got flubbed. Just had a bunch of Fereldens accost me in Low Town, and praise the rule of "our good queen Anora."
Given that I know that my only playthrough of Awakenings went Magi, Romance Alistair, Kick Anora out on her Kiester, Save Alistair with the Dark Ritual, Everyone Lives, Alistair frees the Mages. . . it seems obvious that a game setting, somewhere, went horribly awry.
Any suggestions?
